Adult Medical Day Program
A Program Where L.I.F.E. Begins
We offer an innovative approach to the traditional adult medical day care setting.
The Adult Medical Day Program opened its doors on February 12, 2007. Our fully licensed Adult Medical Day Care program is designed to maximize the independence of the participants and to help maintain or improve their current level of abilities. These services are available to individuals from age 18 and older who may have an intellectual or physical disability, or medical concerns. We also serve Individuals with memory issues.
Participants enjoy a variety of services including medical care, scheduled activity programs, nutritious lunch, snacks, as well as other specialized services. Meals, snack planning and preparation is overseen by our Serv-Safe Certified Food Service Manager who is also a Registered Dietitian. Meals are delivered by a catering service. Special diets are also available.
Medical Care is provided by On-Site Nursing Staff
Who provide ongoing physical assessments, and follow up with all medical care that our participants may require. Our nursing team is also experienced in gastric feeding and provides such services within the medical day program setting. The program offers assistance with scheduling medical appointments. Transportation is also available to medical appointments that occur within Adult Medical Day Care operational hours. The program also has a Medical Director who is a Board-Certified Physician.
Our Social Worker
Provides one on one interaction with families and caregivers. Our social worker assists with the admitting process. Upon admission, each individual acquires his/her own interdisciplinary team, consisting of a nurse, social worker and activities coordinator. This team collaborates to provide the most thorough assessment of all participants and provides continual assistance throughout their day program experience.
Our participants enjoy a wide variety of scheduled activities daily as well as community outings and special event programming each month. Daily scheduled activities include physical fitness, range of motion, sensory stimulation, fine and gross motor activities, Men and Women ‘s Groups, arts and crafts, music programs, cooking groups, grooming, games, computer, and story hour/reading groups. Participants and staff alike are recognized for their birthdays at our Monthly Birthday Celebrations. These activities are supervised by our Activities Coordinator.
Physical Fitness and Health Related Therapies
Our participants enjoy a vast range of physical fitness activities, such as Mall Walks, Community Walks, modified sports and exercise. Under nursing supervision, we provide a weekly assisted walking program for participants who require additional support while ambulating as well. As needed, a Licensed Physical Therapist comes to the center to provide services to our participants. We also offer Occupational Therapy plus speech and language services.
Wheelchair Repair Services
Wheelchair repairs can be scheduled and completed within our facility during regular program hours.
Transportation is provided to and from the Day Program for all participants who live within the catchment area. Hours of operation are Monday-Friday, 8am-4:30pm. Program Hours are 8am-3pm. Departures begin at 2pm.
